Almost identical headlight bulb setup. Wired everything up and had no port 3, driver side high or low, (could plug into 4 and seemed ok) also no plow lights.

The most likely problem is the grounds from the plow lights themselves to the frame. Take the lights apart and check and/or replace the grounds, wires inside the case take a lot of vibration and that can cause them to fail.Don't forget to check the bulbs to be sure they're good too.
